Group & Individual Health Insurance

The Centers are approved for third party payment of mandated and major medical benefits. Services are generally covered through group and individual health insurance policies. In the state of Wisconsin, most standard health insurance policies provide at least $1,800 (effective May 1992) for the treatment of mental/emotional disorders and drug and alcohol abuse problems on an outpatient basis (per person per calendar year). While the patient is responsible for payment of provided services, we will assist the patient in filing claims with insurance carriers for covered procedures. The Centers are also providers for several Health Maintenance Organizations, and accept referrals from each HMO according to its procedures.

We understand that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of the care and services you receive since we need this record to provide you with quality care and to comply with certain legal requirements. This notice applies to all of the records of your care generated by our Centers, whether made by medical center personnel or your personal doctor. Your personal doctor may have different policies or notices regarding the doctor’s use and disclosure of your medical information created in the doctor’s office or clinic.

This notice will tell you about the ways in which we may use and disclose medical information about you. We also describe your rights and certain obligations we have regarding the use and disclosure of medical information.

We are required by law to:

  1. Make sure medical information that identifies you is kept private;
  2. Give you this notice of our legal duties and privacy practices with respect to medical information about you; and
  3. Follow the terms of the notice that is currently in effect.
